PEP, or post-exposure prophylaxis, is a short course of HIV medicines taken very soon after a possible exposure to HIV to prevent the virus from taking hold in your body. You must start it within 72 hours (3 days) after a possible exposure to HIV, or it won’t work. Every hour counts! PEP should be used only in emergency situations.

If taken within 72 hours after possible exposure, PEP is highly effective in preventing HIV. But to be safe, you should take other actions to protect your partners while you are taking PEP. This includes always using condoms with sexual partners and not sharing needles, syringes, or other equipment to inject drugs.

Firstly, that the third drug for PEP appears to be necessary: two drugs work for PrEP but not for PEP. Secondly, starting PEP promptly, ideally within 12 hours of exposure and certainly within 24 hours, really does make a difference to its efficacy. There is also no new evidence that extending PEP beyond 36 hours helps.

If a person who has been exposed to HIV acts quickly to get PEP (within 72 hours) they stand a good chance of stopping the virus from establishing itself in their body. So, if I take PEP I won’t become HIV positive?
https://www.cdc.gov/hiv/risk/pep/index.html
PEP is the use of antiretroviral drugs after a single high-risk event to stop HIV seroconversion. PEP must be started as soon as possible to be effective—and always within 72 hours of a possible exposure.

Randomised studies of PEP have not been done, but experience suggests it is effective in preventing HIV infection. PEP is most effective when it is started soon after exposure to HIV, when doses are not missed and when people complete the 28-day course.
