This was confusing for me, but you just hang on to that form in your personal files. You don't send it in to ASHA and it won't show up on the CE Registry. At the end of your 3-year maintenance interval, or as soon as you have your 30 hours, you can mail or electronically submit your Certification Maintenance Compliance Form. The compliance form is basically just the honor system unless you get audited.
